The Critical Care/Ventilation/Multi-organ Dysfunction Syndrome (MODS) Clinical Domain Team, one of few ?joint? N3C Domain Teams, aims to better understand and characterize complex disease trajectories including multiple organ dysfunction, along with determination of treatment efficacies to improve outcomes in critically ill patients with COVID-19. The vision of the Critical Care/Ventilation/MODS Domain Team is to understand the pathophysiology of the SARS-CoV-2 virus on single and multi-organ systems, and what determines optimal treatment strategies affecting short- and longer-term outcomes of critically ill COVID-19 patients. The ventilation subgroup is focused on defining critical care cohorts based on ventilation strategies, as well as studying clinical outcomes on intubated patients versus those not intubated. The MODS subgroup focuses on characterizing multi-organ dysfunction as a temporal trajectory and probabilistic process.

Both subgroups are actively soliciting members to join and contribute to the goals and mission of this highly unique domain team.

Three protocols have been drafted to examine: (1) Effect of glycemic control on COVID-19 outcomes, (2) COVID-19 outcomes as a function of metformin use, and (3) Comparative effectiveness research on COVID-19 outcomes as a function of antihyperglycemic medication use at baseline. The overall goal is to evaluate baseline factors for effects on COVID-19 outcomes in diabetes.

The initial research will focus on a subset of target populations, including persons with HIV, solid organ transplant patients, and patients with autoimmune disorders, including skin diseases such as atopic dermatitis and eczema. The team will also identify areas of research for the ISC populations that will require additional in-depth study at a level that the N3C data may not be able to provide. Through this research, a better understanding will be gained for how various types, levels, and durations of immunosuppression or compromise contribute towards ISC patient outcomes upon COVID-19 infection.

The Oncology Clinical Domain Team is studying outcomes of cancer patients who contract COVID-19. Cancer phenotypes and concept sets using OMOP data definitions in the N3C Data Enclave are continuously being developed and refined.
